The sign you’re looking for is the one that signals rupture contained within the implant capsule. When the implant shell tears but the surrounding fibrous capsule remains intact, the collapsed shell within the silicone gel creates wavy, undulating lines on MRI that resemble linguine. This linguine sign is the classic, most specific indicator of intracapsular (within the capsule) rupture. If rupture extended outside the capsule, silicone would be seen outside the capsule boundary, indicating extracapsular rupture. In short, the linguine sign directly points to rupture kept inside the capsule.
